CT PHFP Agency Field Placement Proposal Application and Instructions

This application should be completed by a public health agency or fellowship preceptor. An eligible student fellow can be identified before or after the placement is approved.

Agency eligibility requirements:

  • must be located in the State of Connecticut
  • eligible agencies include local Health Departments/Districts, DPH programmatic units, or other state or local government agencies, or community-based public health agency that works closely with the state or local health agencies
  • able to provide a structured fellowship position that involves at least 200 hours of direct public health practice, and
  • provide exposure and work activities related to health equity, essential public health services, and public health careers

This is a great program to help students who don’t have the privilege to take on a fellowship that is not paid because they need to work throughout the school year, and this program allows the student to accept a fellowship that may interest them more.
