Abstract: Stroke is the leading cause of disability and mortality in rural China while implementation of evidence-based practices at the primary care level is inadequate. To address this gap, we conducted a series of research over eight years to develop, contextualize, and evaluate a system-integrated and technology-enabled model of care (SINEMA) for improving primary care-based stroke management in rural China. I will introduce both formative and summative research guided by theories, models, and frameworks and approaches in implementation science including the design of a hybrid type 2 effectiveness-implementation trial and its long-term follow-up to evaluate sustainability. I will conclude by sharing lessons learnt that can be applicable to other multi-component complex health interventional implementation research projects.
