Biography

Dr. Archana Shrestha, PhD, MPH, is a Professor and Director of the Public Health Program at Kathmandu University School of Medical Sciences (KUSMS) and an Adjunct Associate Professor at the Yale School of Public Health. She directs the Institute for Implementation Science and Health, where she designs and leads capacity-building programs that strengthen translational research and implementation science in Nepal. Dr. Shrestha completed a Postdoctoral Fellowship in Implementation Science at Harvard T.H. Chan School of Public Health (2018), earned a PhD in Epidemiology from the University of Washington (2015).

Dr. Shrestha’s research integrates epidemiology and implementation science to strengthen prevention and care for noncommunicable diseases and related maternal and youth health priorities in resource-limited settings. Her work advances community- and health system–based strategies for hypertension and diabetes prevention and control, improves diet and cardiometabolic risk through workplace and community interventions, and develops digital and task-shared models to increase access, quality, and continuity of care. As a multiple principal investigator, she leads several NIH-funded studies, including Community Health Worker–led Hypertension Prevention and Control in Nepal (R01HL169421; 2023–2028, mPI Spiegelman), Mobile-based Obstetric Monitoring for Pregnancies Complicated by Hypertension and/or Diabetes (MOM-HD), a Type II Hybrid Implementation-Effectiveness Trial (R01HL176741; 2025–2030 mPI Rawal ), Innovations for Peer-delivered and Family-engaged Brief Interventions for Youth Suicide in Nepal, a pilot hybrid type II implementation study (R34MH136250; 2024–2027 mPI Hagaman), and a game-based mobile intervention to limit excessive gestational weight gain and postpartum weight retention among pregnant women with overweight and obesity (R21TW012615; 2025–2027 PI Rawal). In addition, she contributes as Co-Investigator to large multi-site hybrid effectiveness-implementation research, including BECOME— a Type II hybrid implementation-effectiveness study integrating a behavioral community-based combined intervention for mental health and NCDs delivered by community health workers (R01MH133231; PI Acharya; 2023–2028), SCALE-NCD—scaling up community-based NCD research into practice in Pokhara Metropolitan City (R01HL172271; PI Neupane; 2024–2029), and an implementation pilot integrating a suicide prevention package into decentralized primary health care systems in rural Nepal (R34MH135122; MPIs Hagaman and Sapkota; 2024–2027). Beyond NIH funding, she serves as PI for AMPATH–Nepal cervical and breast cancer programming (Lily Foundation; 2023–2026) and for AMPATH–Nepal group care model development and testing (Lily Foundation; 2024–2027), is a Co-Investigator in the NIHR Global Health Research Centre for Multiple Long-Term Conditions (NIHR203257; PI Prabhakaran; 2022–2027), and leads a Resolve to Save Lives' grant to develop, implement, and evaluate a healthy food procurement policy in Nepal (2020–2025); and leads WHO Health Alliance grant on "Implementation Research for integrated management of Diabetes and Hypertension (2024-2027)

Dr. Shrestha’s contribution to science lies in generating context-specific epidemiologic evidence and translating it into scalable, implementation-ready interventions for low- and middle-income countries. She has advanced hybrid effectiveness-implementation methodologies in chronic disease and maternal health, strengthened task-shared models of care delivered by community health workers, and integrated behavioral and digital strategies into primary healthcare systems. Through longitudinal cohort studies, cluster randomized trials, systematic reviews, and policy-focused implementation research, she has contributed to understanding cardiometabolic risk, health system gaps, stigma, service utilization, and digital monitoring approaches in Nepal and similar settings. Her work bridges rigorous epidemiologic inquiry with real-world health system application, contributing to the global evidence base on equitable, scalable prevention and management of noncommunicable diseases and mental health conditions.

Overview

Community Health Worker–Led Hypertension Prevention and Control in Nepal (NIH-funded) This implementation trial evaluates a task-shared model in which community health workers deliver hypertension screening, treatment support, and behavior change counseling. The study applies implementation science frameworks to assess effectiveness, fidelity, cost-effectiveness, and sustainability while strengthening primary healthcare systems for chronic disease management.

Mobile-based Obstetric Monitoring for Pregnancies Complicated by Hypertension and/or Diabetes (NIH-funded) This Type II hybrid effectiveness-implementation trial evaluates a mobile health–enabled monitoring system designed to improve detection, follow-up, and management of hypertension and diabetes during pregnancy. The study simultaneously examines clinical outcomes and implementation outcomes, including adoption, fidelity, reach, and sustainability within routine maternal health services.

Innovations for Peer-delivered and Family-engaged Brief Interventions for Youth Suicide in Nepal (NIH-funded) This pilot hybrid implementation study develops and tests peer-delivered and family-engaged interventions to reduce suicide risk among youth in Nepal. The project integrates community-based behavioral approaches into decentralized primary healthcare systems and evaluates feasibility, acceptability, and preliminary effectiveness to inform scalable suicide prevention strategies.

AMPATH–Nepal Cervical and Breast Cancer Program (Lily Foundation-funded) This program strengthens cervical and breast cancer screening, early detection, referral systems, and community engagement in Nepal. The initiative integrates provider training, system-level improvements, and community mobilization to improve uptake and continuity of care across the cancer prevention and treatment cascade.

AMPATH–Nepal Group Care Model Development and Testing (Lily Foundation-funded) This project develops and evaluates a structured group-based care delivery model to improve maternal and chronic disease outcomes. Using participatory design and implementation research methods, the study assesses patient engagement, peer support mechanisms, and health system integration to enhance service delivery efficiency and quality.

Develop, Implement, and Evaluate Healthy Food Procurement Policy in Nepal (Resolve to Save Lives-funded) This policy implementation study designs and evaluates institutional healthy food procurement standards to improve dietary environments and reduce cardiometabolic risk at the population level. The project generates policy-relevant evidence on feasibility, adoption, and sustainability of structural nutrition interventions within public sector institutions.

Better Nutrition for Adolescents (Norwegian Research Grant–funded) The Better Nutrition Project is a school-based intervention and implementation research initiative designed to promote healthy weight among adolescent school-going children in Nepal. The project develops contextually adapted nutrition and physical activity interventions alongside theory-informed implementation strategies to improve adoption and sustainability within school systems. The intervention engages students, parents, teachers, and municipal stakeholders to co-design multicomponent strategies targeting dietary behaviors, physical activity, and the school food environment. The study evaluates effectiveness outcomes such as BMI, diet quality, and physical activity levels, and assesses implementation outcomes using the RE-AIM framework, including Reach, Effectiveness, Adoption, Implementation fidelity, and Maintenance. The project aims to generate scalable, policy-relevant evidence to prevent early cardiometabolic risk and strengthen school-based health promotion in low-resource settings.
